Thanks for the various suggestions.

Based on a suggestion from private mail I chose 'OrderedMultiDict'.
I've also implemented 'UnorderedMultiDict' which has weaker guarantees
about order, at the advantage of better deletion performance.  They
are both available in a module called 'MultiDict'.
