LGTM! A couple of thoughts that you can take or leave:

1. The options for where to get help should probably be ordered by preference, and I'd prefer someone schedule an office hour with me before going to the dev lists or filing a bug.

2. The lines of the table are a bit long; they wrap in the GitHub review view by 6 or 7 characters. Shortening them could make the table longer vertically and make adding new entries more annoying.
